"Vaccinate Before You Graduate"
The Jennings County Health Department will be at the high school on the following dates to give immunizations. This is part of Indiana's "Vaccinate before You Graduate" program.

From 8 to 12 & 1 to 4
The clinic days will focus on Tetanus and Hepatitis B for the freshmen and senior classes. Screening Forms and Parent / Guardian Permission Slips must be filled out, signed and returned to the school nurse prior to the designated dates.
Vaccine information sheets are also downloadable on this site. Please read this information prior to completing the forms.
NO immunizations will be given unless the paperwork has been completed and returned to the school nurse. Our goal is to increase adolescent immunizations. Please contact your school nurse or public health nurse for any questions.
